Nursing Home Administrator Consultant – My business is to provide services to the long term care community with the proven ability to reorganize troubled facilities. As a successful Administrator/Executive Director I am able to reintroduce a high quality of patient care in skilled facilities by the use of the Quality Assurance program. The tracking by the Quality Assurance Committee of implemented systems will assure that the facility will be brought back into sustained compliance with State and Federal regulations. Park Regency Care Center La Habra, CA - 99 bed SNF Administrator September, 2005-April, 2007 I was the Administrator responsible for facility operations. We developed and implemented a strong and successful marketing strategy. The facility had a successful DHS survey. There was increased community physician participation in the facility. The facility met its’ budgetary goals and improved the quality of resident care with increased Medicare census and increased utilization of Medicare Part B benefits.

Arlington Gardens Care Center Riverside, CA - 28 bed SNF Executive Director December 2004-August, 2005 As the Executive Director responsible for facility operations, there were many successes for this historically poor performing facility. My plans called for the successful execution of Profit and Loss management, creating departmental operational budgets, marketing and sales, leadership/team building and resident and family satisfaction. The facility was stabilized and negative operational and financial trends were reversed. The facility had increased net revenues. A comprehensive rehabilitation program and post acute services were implemented. Managed care contracts were successfully negotiated and strong discharge planning relationships were established with the acute hospitals, physician and case managers. A Risk Management program was designed and implemented which resulted in a nine month period of time with no loss injury days. The facility exceeded profit, operational service goals were met and a continued 97% occupancy rate was maintained. I was designated as a Peer Assistant to the corporation’s three other facilities and provided operational assistance with my expertise, as my goal of restoring strong operational systems and turning the facility into a highly progressive and successful entity had been met.

Candlewood Care Center Long Beach, CA - Candlewood Care Center Long Beach, CA 120 bed SNF Interim Administrator November, 2003-July, 2004 As the interim Administrator I was responsible for facility operations until the transition of the facility to the new owners. Financial budgets were met. The census was maintained at 95% and facility had a successful survey. It was found to be in substantial compliance in July, 2004.

Carriage House Nursing Center Fullerton, CA 99 bed SNF Administrator April, 2002-May, 2003 AS the Administrator I was responsible for facility operations. Budget obligations were met monthly. I developed an effective marketing strategy for census management. The facility developed and implemented an Alzheimer’s Secured Unit with 22 dedicated beds and has been operational since May, 2003. The facility had a successful DHS survey for the first time in a five year history.

Sun Mar Nursing Center Anaheim, CA 69 bed SNF Administrator March, 1998-April, 2002 As the Administrator I was responsible for facility operations. Our budget goals were met monthly. I was instrumental in the development and implementation of a Korean Cultural Program in July, 1998. I developed the facility’s marketing strategy by working closely with physicians and hospitals in the community. We initiated a “Falling Star” program which enabled the facility’s fall incidents to decrease by about 90% in the first quarter after initiation. The facility had a Deficiency Free DHS Survey in February, 2002.

Anaheim Health Care Center, Anaheim, CA 218 bed SNF Admission Coordinator Alzheimer Unit and Social Services Designee (SSD) July, 1997-March, 1998 I was responsible for admission procedures and discharge planning for residents in the Alzheimer Unit. As the SSD I was responsible for meeting the resident’s psychosocial needs with appropriate interventions. I was then transferred to the Sun Mar Nursing Center as the Administrator.
